These clinics are by appointment and are held by the practice nurses.

Travel vaccinations

Please complete a travel risk assessment form at least 6 weeks before you travel.

The form will be reviewed by the practice nurse who will check your medical records to identify which of the requested vaccines you need.

You will contacted by SMS or phone to arrange an appointment with the practice nurse for the appropriate vaccinations.

It is important to complete the form as early as possible. These vaccines have to be ordered as they are not a stock vaccine. For all non-NHS vaccines there will be a fee and all payments will need to be paid at the time of ordering.
